Assessment Team
We aim to complete timely assessments to ascertain whether adults have eligible social care needs.
Review Team
We aim to undertake thorough, meaningful and timely reviews of adults with eligible social care needs who are supported to live in the community and those who are placed in residential and nursing care homes.
DoLS Team
The DoLS Team is based at the newly refurbished Civic Offices in Milton Keynes. It is made up of a variety of experienced Best Interest Assessors and Community Liaison Officers.
Reablement and Hospital Discharge Team
We are a fast paced team where no day is the same. We have two functions in our team which includes supporting safe and timely discharges from hospital and supporting people to be discharged from our reablement and recuperation services in the community. We are a team that embraces challenge and our key ethos is supporting people to maximise their independence in their own homes. We are very fortunate to work as part of a joint health and social care team and therefore are able to collaboratively work together to meet the needs of the people we work with.
Community Learning Disability Team
We are a fully integrated multi-disciplinary Community Learning Disability Team. We support people with a learning disability to live their lives as independently as possible and support them to reach their full potential. We are looking for people with experience in learning disability and mental health but would like to hear from anyone with experience in either client group. We support a variety of people with a diverse range of outcomes including those with complex needs meaning that our work is interesting, challenging and variable on a day to day basis.
Mental Health and Autism Team
We are a growing and developing team, and our work is challenging but rewarding. As a social worker in the team you will be responsible for completing Care Act Assessments and reviews. We provide social supervision and produce social circumstance reports. You will be part of a duty rota, and will carry a case load of often complex cases. Our emphasis is on enhancing the recovery and wellbeing of those that access our service.
